F3 Personal History Statement

After successful completion of the assessments, you will be provided with the Sheriff Education & Training Standards Division form F-3 Personal History Statement and required documents.  A team member will review your packet when you come in to drop it off.  We will go over your paper work with you to ensure that you have filled it out completely and correctly to ensure a smooth start. We have several notaries on staff that will notarize all your documentation.

Documents Needed

  • Valid Driver's License or Photo ID

  • Birth Certificate or Naturalization Document

  • Signed Social Security Card

  • Completed Educational Diploma or Sealed Official Transcripts

  • Prior military: member copy DD - 214 (must contain separation code at bottom)

  • Court documents of any criminal charges (does not include minor traffic offenses) or civil actions such as bankruptcy or repossessions
